Losing a fish is miserable, and when it keeps happening it is easy to decide you are simply unlucky, or that the hobby is not for you. In almost every case I have been asked to help with, the fish were not weak and the shop was not selling duds. Something in the tank or the routine was quietly wrong, and once it was found the deaths stopped. This is the checklist I work through with people, roughly in the order the problems tend to show up.

Start with the water, not the fish

Fish live in a closed box of their own waste, and the things that kill them are usually dissolved and invisible. That is the hard part for a new keeper: you stare at the fish looking for a torn fin or a white spot, when the actual killer has no colour or smell at all. So before you change anything, you need to know what the water is doing.

Get a liquid test kit rather than relying on the paper strips, which are fine for a rough steer but drift out of accuracy quickly. Test ammonia, nitrite, nitrate and pH, and write the numbers down. If you have never tested your water, that single purchase will tell you more than any amount of guessing. New tank syndrome, the number one killer

If your tank is less than a couple of months old, this is the prime suspect and it is worth explaining properly. A brand new aquarium has almost none of the bacteria that process fish waste. Fish produce ammonia constantly, and in an uncycled tank it has nowhere to go, so it climbs. Ammonia burns gills at concentrations as low as a quarter of a part per million. As the first bacteria appear they convert that ammonia into nitrite, which is just as toxic and stops the blood carrying oxygen, so the fish effectively suffocate in clear water.

Growing enough bacteria to keep pace takes roughly four to six weeks. Add a full load of fish on day one and many of them will not survive the wait. If you are already in this situation, keep the fish alive with daily testing and water changes large enough to hold ammonia and nitrite near zero, use a dechlorinator on the fresh water, and stop adding livestock. One point that matters especially here, because this site is built around very hard alkaline tap water. Ammonia becomes more poisonous as pH and temperature rise, because more of it takes its dangerous free form. So the same ammonia reading that would be survivable in soft, acidic water can be lethal in a warm tank at pH 8. An old tank that suddenly turns

When a tank has run happily for months and then loses fish overnight, something has changed, and your job is to find what. The usual triggers are a filter clean that went too far and wiped out the bacteria (media should be rinsed in old tank water, never under the tap), a water change where the dechlorinator was forgotten, a fish or snail that died unseen and is rotting behind the rocks, a spell of heavy feeding, or a heater that has stuck on or failed off. Walk backwards through the last week and something usually stands out.

Chlorine and chloramine from the tap

Tap water is treated to kill bacteria, and it does its job indiscriminately. Pour it straight into a tank and it damages the fish’s gills and can knock back the filter bacteria you have spent weeks growing. Chloramine, which many water companies now use instead of plain chlorine, is more stubborn and will not simply gas off if you leave a bucket standing overnight. Always add a dechlorinator, dosed for the volume you are replacing; the volume calculator will tell you how many litres that actually is if you have never measured. Refilling an entire tank directly from the tap is a classic and completely avoidable wipeout.

Temperature shock and rushed acclimatisation

Fish are cold blooded and cope badly with sudden swings. The most common self-inflicted version is tipping a bag of shop fish straight into the tank, hitting them with a change in temperature and water chemistry at the same moment. Float the bag to match temperature, then add tank water to the bag in stages over half an hour or so before you net the fish across. A heater that is faulty or the wrong size for the tank causes the slower version of this problem, with the temperature drifting up and down over days; the heater calculator will size one correctly. And tropical fish kept in a cold room, or a tank with no heater at all, simply run down over time.

Too many fish, added too fast

Every fish adds to the waste load, and a young filter can only process so much. Stock a new tank slowly, a few fish at a time with a fortnight between additions, so the bacteria can catch up before the next lot arrives. Overfeeding does the same damage from the other direction, since uneaten food rots into ammonia. Disease that came in with the fish

Even with faultless water, new fish can carry ich, velvet or internal parasites, picked up in shop tanks that often share one filtration system across dozens of aquariums. One sickly fish then seeds the whole display. This is why a quarantine tank earns its keep, letting you watch and treat new arrivals in isolation for a fortnight before they meet your established stock. The wrong fish for your water

This one is slower and easier to miss. If your tap water is very hard and alkaline, as ours is at around 22.5 dGH, then soft-water species such as many tetras, wild-caught bettas, dwarf cichlids like rams and most delicate blackwater fish will rarely thrive. They do not usually drop dead in a week; they just never look quite right, colour up poorly, and die younger than they should while you wonder what you did wrong. The kinder and easier route is to keep fish that suit the water coming out of your tap. Working through it without panicking

When you are in the middle of losing fish, the instinct is to throw every remedy at the tank at once, which usually makes things worse. Slow down and go in order. Test the water first and act on ammonia or nitrite before anything else. Do a dechlorinated, temperature-matched water change, since it rarely hurts and often helps. Check the heater and the dechlorinator. Review how many fish you have and how much you feed. Only then start thinking about disease and medication. Most of the time the answer is dull and fixable, and the tank that has been breaking your heart settles into the quiet, steady thing it was always meant to be.
